Terms & Conditions

Financial Responsibility of the client:

The client is considered responsible for payment of professional fees. It is the client’s
responsibility to determine if services are covered and to understand their deductible
and/or co-payment amount (if utilizing insurance benefits). When we are asked to bill a
third party, such as a divorced spouse, relative, or insurance company, and that third
party fails to make timely payments, payment is expected from the client who signed the
consent for services. The client will be responsible for fees associated with claims that
are denied (e.g., due to exceeding the number of available sessions, if new coverage
has not commenced, or if insurance has changed, or if filing occurs past the insurance
carrier’s time limit, etc.).

Insurance Information:

I am contracted with several private insurance companies, including Anthem Blue Cross
Blue Shield, Aetna, Cigna, UnitedHealthcare, and Optum (Oscar and Oxford). Currently,
I do not accept Medicaid.  

 

HSA/FSA Eligible:

Therapy services qualify for payment through Health Savings Accounts (HSA) and
Flexible Spending Accounts (FSA).

Payment Methods:
If you choose to pay out of pocket, credit cards are the only accepted form of payment.
You will be required to provide a credit card on file before your first
scheduled appointment. Your card will not be charged until your scheduled session
begins, except in the event of a late cancellation or a missed appointment (no-show) for
the scheduled appointment. Unfortunately, I do not accept personal or business
checks.To make therapy more accessible, a limited number of sliding-scale spots are available
for clients experiencing financial hardship. Please inquire about current availability.

Cancellation and No-Show Policy:

A rate for a full session will be charged to the credit card on file for all late cancellations
(cancellations made less than 24 hours before the scheduled appointment) and missed
appointments (no-shows).  


Past Due Accounts:
We send written correspondence to clients with past-due accounts to provide an
opportunity to pay in full or establish a payment plan. Suppose a client has not made
good faith efforts to pay their bill. In that case, the overdue account will be assigned to
our collections agency, and all collection costs associated with the debt will be the
client’s responsibility. We also reserve the right to assign the account to small claims
court, depending upon the total balance due.
